Health conditions & medication

Medication at school.

Students with asthma, epilepsy, diabetes, fatal allergies that require an EpiPen, or ADHD may need to have prescription medication with them at school. If your child needs medication on campus, a few steps must be completed before the first day of school.

Medication at School: A Guide for Parents

Preschool families: find the children’s medical report and medication forms on the Preschool page.

The 30-day grace period

North Carolina allows a 30-day grace period: students may attend school up to the 30th day from their start date. After the grace period, students who have not met the immunization requirements may not attend school until the requirements are met.

For more information, visit the NC Department of Health and Human Services.

Where to get vaccines

If your child’s annual exam is already done but vaccines are still needed, a vaccines-only appointment can often be made with your child’s doctor’s office. You can also visit:

  • New Hanover County Public Health Department 1650 Greenfield St, Wilmington, NC 28401 · (910) 798-3500
  • Sam’s Club
  • CVS MinuteClinic

Families who choose to decline vaccinations because of religious beliefs may file a Request for Religious Exemption from Immunizations form.

Health requirements for kindergarten

Before your child starts kindergarten.

In North Carolina, kindergarten students must meet these healthcare requirements before beginning school. North Carolina allows a 30-day grace period for this documentation.

  1. Birth certificate

A copy of your child’s birth certificate on file with the school.

  2. Health assessment

A health assessment within 12 months before starting kindergarten, with information about medications, allergies, developmental concerns, and special diets. A licensed healthcare provider, such as a physician, physician’s assistant, certified nurse practitioner, or public health nurse, must conduct it, and the NC Health Assessment Transmittal Form must be signed during or after the physical exam.

  3. Immunization record

A completed immunization record on file by the 30th day of attendance, including the kindergarten vaccines listed above.
